The Protection and Use of Patient Information — Confidentiality

 

We ask you for information so that you can receive proper care and treatment.

 

We keep this information, together with details of your care, because it may be needed if we see you again.

 

We may use some of this information for other reasons:  for example, to help us protect the health of the public generally and to see that the NHS runs efficiently, plans for the future, trains its staff, pays its bills and can account for its actions.   Information may also be needed to help educate tomorrow’s clinical staff and to carry out medical and other health research for the benefit of everyone.

 

Sometimes the law requires us to pass on information:  for example, to notify a birth.

 

The NHS Central Register for England and Wales contains basic personal details of all patients registered with a general practitioner.   The register does not contain clinical information.

 

You have a right of access to your health records—you can do this by contacting us.

 

EVERYONE WORKING FOR THE NHS HAS A LEGAL DUTY TO KEEP INFORMATION ABOUT YOU CONFIDENTIAL

 

You may be receiving care from other people as well as the NHS.   So that we can all work together for your benefit we may need to share some information about you.

 

We only ever use or pass on information about you if people have a genuine need for it in your and everyone's’ interests.   Whenever we can, we shall remove details which identify you.   The sharing of some types of very sensitive information is strictly controlled by law.

 

Anyone who receives information from us is also under a legal duty to keep it confidential.

 

The Practice computer system is part of the National Programme for IT. It is linked to a mainframe computer system offsite and connected to the NHS Spine. The Spine is part of the NHS Care Records Service (NHS CRS). A key part of the NHS CRS will be the gradual development of an electronic Summary Care Record. This will eventually be available to NHS Staff involved in your care, anywhere in the country, and you will be able to view a summarised version of it yourself.

 

 

Suggestions and Complaints

 

The Practice strives very hard to ensure that everyone receives quality care, and welcomes any suggestions or comments from you as how to improve the service we offer. Should you feel that your needs are not being met, we welcome the opportunity to discuss this with you.

Please contact the Practice Manager who will be happy to deal with your complaint, either by telephone, letter or calling in at the surgery.

 

If you remain dissatisfied with the outcome of the practice investigation, you can contact the Practice Manager or the Primary Care Trust’s Complaints Manager on 01327 708 100 as soon as possible, who will be able to advise you further.

 

If, after this, you are still dissatisfied with the response to your complaint, you have the right to ask the Health Care Commission (who are a national independent body set up to improve healthcare) to review your case.  You should do so within 2 months from the date of the final response letter from the practice.   You can contact the Healthcare Commission Complaints Team on 020 7448 9100, or visit their website, www.healthcarecommision.org.uk.

Patient Participation Group

 

We have an active Patient Forum, and would welcome more volunteers to be part of the group. The group aims to provide feedback and join in general discussions on how we can provide the best service to our patients.

 

We meet every two months, and feel it’s very important to have our patients’ input and opinions. Please contact Ann Waters if you are interested in joining the group.

Moving out of our practice area

 

Sometimes patients move out of our practice area, but wish to stay registered with our practice.

 

We are very sorry for the distress and inconvenience caused, but we are unable to continue to offer our services in this case. You will need to register with a new GP near your new house. This is standard practice across all GP practices.
